A Clinical Usefulness of Quantitative Antigen Test for COVID-19 Diagnosis

背景：現在SARS-CoV-2 検出には，RT-PCR 検査が最も検出感度に優れていると言われている．一方，抗原定量検査は，各医療機関でまだ十分に利用されておらず，信頼のおける報告はほとんどない．今回，当院で施行した抗原定量検査とRT-PCR 検査の結果から抗原定量検査の有用性を検討した．方法：2020年8月13日から9月12日の間に，抗原定量検査とRT-PCR 検査を同時に施行した外来および入院患者115名149検体を対象とした．①抗原定量検査とRT-PCR 検査の検出率②結果が不一致となった症例の属性の比較③発症からの日数と抗原濃度の相関性について検討した．結果：①抗原定量検査とRT-PCR 検査の陽性一致率は100％，陰性一致率は88.4％，全体一致率90.4％であった．②年齢，男女比，発症から検査までの期間，検査時体温，肺炎像の有無に明らかな差はなく，抗原濃度測定値の中央値は陽性一致群（M 群）5,000pg/mL，いずれかの結果が陰性であった不一致群（U 群）74.88pg/mL で，M 群が有意に高かった．（p=0.002）③発症からの時間経過とともに抗原濃度は減少する傾向（相関係数r=－0.656）であった． 結論：SARS-CoV-2 抗原定量検査は，RT-PCR 検査と比較して，初回スクリーニングから臨床経過の全体を通して，十分な検出感度があり，スクリーニングを含めた感染制御の場面および臨床経過の評価に有用である可能性を示した．
